The state of Pennsylvania requires all third (3rd) and seventh (7th) grade students to have a dental exam report on file.   Please download the Dental Exam Form here and take with you to your dentist.

The state of Pennsylvania requires all sixth (6th) grade students to have a physical exam report on file.  Please download the Private Physician's Report Form here. 

The state of Pennsylvania requires all students requiring medication during school hours to have an authorization form on file.  Please download the   Authorization for Medication During School Hours Form here and have your physician complete.
